Your medical records are confidential and you may approve or refuse the release of medical information to anyone outside the clinic, except when required by law. For information regarding your medical records, please call the clinic at 763-535-9601, extension 240, or you may fax your request to 763-535-5601. Please allow two weeks to receive your medical records. |
