Insurance & Billing

We encourage you to talk to us if you have questions about insurance or the cost of your care. We will file insurance claims for you if we are provided with complete billing information. You will be notified of any balance due after payment and adjustment are made. You are responsible for paying any deductible, coinsurance comapny. If you have questions about your bill, please call our billing service number at 1-800-645-6554.

You will be asked to present your current insurance card(s) at each visit. Failure to present your current card(s) will release New Hope Crystal Medical Clinic from any responsibility for incorrect or untimely filing of contracted claims.

New Hope Crystal Medical Clinic is a participating provider with many local health plans and major insurance companies. I you have questions about your coverage, call the number listed on the back of your insurance card or contact your employee benefits department. For those patients without health insurance, we offer a 15% discount at the time of service.
